Been trawling the internet for answers to my question.
I am self-employed for 5 years, earn less than £5000 a year so don't pay tax, pay voluntary N.I and haven't a small earning cert and therefore have no payslips.
When I sent off my MA1 form and maternity cert is that it? Do they need proof from my bank account of monthly income?
Don't want to send it all off to then have to do it again!
been on the Direct Gov site and have read the MA1 form but still confused...help!

Yes, it is very confusing isn't it?
I was self employed (just starting my business) and also employed for a time during the 66 week test period.
I had to pick my best 13 weeks for them to base my payments from. Like you, I was a bit confused. I submitted info from Self Employment and also my payslips from working. I didn't pay my earnings regularly into my bank account from my business as I used it to buy things or to live off generally. I just used to keep a note of everything in my accounts.
They could have taken all the info from me being self employed because I had paid my Class 2 NI payments weekly since starting Self Employment and I didn't have to prove any earnings. As long as you have paid your Class 2s they assume you have enough to get Maternity Allowance so don't worry and you should get it at the top rate. The Department of Work and Pensions who pay MA will get the info they need from HMRC. I hope this helps and outs your mind at rest,x
